How do I apply for Learning Credits?

To apply for a Learning Credits certificate you must first have been accredited to deliver the CDSMC (Chronic Disease Self-Management Course, also know as the Generic Course). You then go to the Learning Credits page which can be found by clicking on the following link http://www.selfmanagement.co.uk/learning-credits. this page can also be found by clicking on 'The National Register of Self-Management' link on the left hand side of this page and scrolling down and clicking on the 'Learning Credits' page.

On this page you will find an online form that you can complete. If you would rather you can also down load the form from by clicking here. Or alternatively, if you contact us with you name and address we can post you a form to complete. The only other requirement is that you are a member of The National Register of Self-Management as it is a benefit to those that are members.

How do I find out if I am on The National Register of Self-Management?

We have a page on our website called 'Find a Tutor'. As well as finding a tutor the form can also be used to find out general information if you have any questions or queries. Use this link http://www.selfmanagement.co.uk/request-information-national-register-self-management and complete the online form. Alternmatively, please feel free to contact the team by emailing admin@selfmanagement.co.uk or cal 03333 44 1800.

How do I update my details?

It is important that you let us know if your details change in anyway. This can be changes such as a change in address, further training taken, being accredited at another level to what our records show or if you attend a supervision or receive a monitoring visit. For some relevant events the organisation running them will inform us on who attended however this does not happen for all events. Therefore, to let us know please email admin@selfmanagement.co.uk

How do I search for a tutor to deliver a course?

 We are a list of tutors etc that are will to increase their delivery experience. If you are short of a tutor or need a Lead Trainer or Assessor to carry out some work then let us know. How do I set up a User Account?

Setting up your account for the first time

  • Go to the 'Login' box at the top left of the page and click on 'Create new account'
  • Decide on a username and add it to the 'Username' box. This can be your name or something that you will easily remember. For example 'Pauline1' or Brightonassessor. Your username will be displayed as a signature to any comments left by you.
  • Then add in your email address. This must be a current, valid email address. If you change your email address don't forget to update us!
  •  Add in your name details. This information is kept private and will not be published anywhere on the site.
  • Then click  'Create New Account'
  • You will then be sent an email containing a system generated password and a the link to activate your account. Click on the link and use the given password
  • You will then be asked to change your password to something more memorable
  • Following that, you are ready to go and starting posting!

What happens if I forget my password?

Firstly, if you forget your password, don't worry. Go to the forum page and just below where you would usually login you will see 'Create new account' and 'Request new password'. Click 'Request new password'. Type in either your Username or your email address and then click 'Email new password'. Go to your email account and there you will find an email from admin@selfmanagement.co.uk. Open the email and click on the link that it tells you to. This will be the second link in the bidy of the email, not the first. You will then be take back to the website to a page called 'Reset Password', click 'Login' and your account will appear. You will see your Username, email address and a place to reset your password. Once you have reset your password you are set to go again.

How do I use the discussion forum?

  • Click on the Forum that you would to contribute to
  • Click again on the topic
  • Click 'Post reply'
  • A 'Comment' box will appear. Use this box to add your post
  •  If a 'Comment' box does not come up try refreshing the page.

What do I do if I see something written in the discussion forum that I find offensive?

The disucssion forums are only moderated on a very casual basis. Therefore, there may (rarely) be an offensive ststement on there. While we hope that this does not happen there may be an occasion if it does. Please be aware that people while people are free to express there opinions. If however you see something that you feel crosses a line then please let the team know as soon as you can. Either email us at admin@selfmanagement.co.uk or call us on 03333 44 1800.

How do I use the 'Links' page?

The links page is organised alphabetically. Therefore when you first see the links page you will only see those links/companies that begin with A. You will see at the top of the page a line of letters in alphabetical order. Some letters may be missing. If you are looking for a The Heart Foundation for example. Go to the letters at the top of the page and lick on the letter H. This will take you to all the links beginning with that letter. It i sthe same for the rest of the letters. For those organisations starting with 'The' the second work supercedes the 'The'. Going back to the example of 'The Heart Foundation' you would look under H for Heart rather than T for The.
